New Patients need this paperwork completed before their first appointment. Failing to complete the correct paperwork will delay the start of your appointment and could reduce the time you spend with your Practitioner. Please be sure to fill out the correct paperwork according to the type of appointment you have scheduled. For example, a Naturopathic visit requires the Naturopathic paperwork. Generally, only one set of paperwork is needed.
